Client Consent to Treat & Waiver, Release, and Assumption of Risk
Dr. Enid F. Martinez is an intuitive healing professional and provides intuitive information for healing work, intuitive consulting, and professional mentoring.
I understand that the therapies provided by Dr. Enid F. Martinez are gentle, integrative, energy-based approaches that support the body’s natural ability to
heal. These services may be conducted in person, as per available space, using light or non-contact touch, or remotely via telephone or Zoom. I acknowledge that these therapies are complementary in nature and are not intended to replace any
medical treatment or advice prescribed by my licensed healthcare providers.
I understand that Dr. Enid F. Martinez is not a licensed medical practitioner in the state of New Jersey. I agree that I am solely responsible for seeking medical advice and treatment from a qualified medical professional when needed.
I agree to actively participate in my healing process and take personal responsibility for my health and well-being.
I understand that intuitive consulting is not a substitute for professional counseling, psychotherapy, or medical advisement.
I understand that all session notes and personal experiences shared during sessions are confidential and will be protected, subject to standard exceptions required by State or Federal laws and regulations.
Cancellation Policy
I acknowledge and accept the 48-hour cancellation policy. Appointments must be canceled or rescheduled with a minimum of 48 hours’ notice. Cancellations made within 48 hours of the scheduled time will incur a fee of 50% of the total session cost. The cancellation fee for a regular 1 hour visit is $110. No-shows are subject to the full session fee. Exceptions to this policy may be made on a case-by-case basis for emergencies or unforeseen circumstances. Your cooperation is appreciated, as timely cancellations allow others the opportunity to book appointments.
Last-Minute and Weekend Appointments
Occasionally, last minute and weekend appointments may be available, Dr. Enid F. Martinez makes every effort to accommodate last-minute appointment requests. These are billed with an additional fee of $100.
Office Hours
Tuesday – Thursday 10:30am – 5:00pm
Friday 10:30am – 2:30pm
Saturday 10:30am – 2:30pm
Sunday and Mondays are non-office hours. After hours and emergency hours are considered outside of above parameters and will be charged and extra $100.00 of the regular fee. I understand that Dr. Enid Martinez will accommodate accordingly depending on her availability.
Refund Policy
I understand no refunds are offered.
Consent to Services
I give my informed consent to receive energy healing consulting, energy-based surgical consulting, and professional mentoring from Dr Enid F. Martinez.
Covenant Not to Sue
I acknowledge I am voluntarily participating in a paid or unpaid session, service, class, workshop, or program with Dr. Enid F. Martinez and/or The Heart Helix, LLC. This includes, but it is not limited to, energy healing consultations (including vibrational medicine, intuitive coaching, e-mail readings), surgical support consultations, and participation in workshops or classes.
In consideration for being allowed to participate, I hereby hold harmless The Heart Helix, LLC, and its agents, employees, representatives, heirs and assigns, from any and all claims, demands, damages, or causes of action – present or future – that may arise out of or relate to my participation in any of the above activities, including but not limited to advice given, training received, or use of facilities.
I understand and acknowledge that no legal, medical, or licensed therapeutic advice is provided by Dr. Enid F. Martinez or any representative of The Heart Helix, LLC. I accept full responsibility for consulting appropriate licensed professionals as needed.
I understand and accept that participation in these services involves certain inherent risks, and I assume full responsibility for any outcomes or experiences resulting from my participation. I acknowledge that no guarantees or warranties have been made regarding specific outcomes, and that individual results may vary.
By signing this document, visiting this website, reading this agreement, or receiving services from Dr. Enid F. Martinez and The Heart Helix, LLC, I acknowledge that I have read, understood, and voluntarily agree to the terms outlined herein. I waive any right I or my legal representatives may have to bring legal action against The Heart Helix, LLC, its employees, agents, or contractors, including for acts of ordinary negligence.

I live in New Jersey, USA in the Eastern Time Zone (same as New York City); any appointment times offered will be in this time zone unless otherwise indicated.
Appointments over Zoom
Remember appointments are over Zoom, or phone unless other arrangements are made. I’m happy to give you a phone number if you prefer or keep our videos off if that makes you more comfortable. Please let me know if any accommodations are needed.
Cancellation Policy
As outlined in your Treatment Consent Form, we require a minimum of 48 hours’ notice for appointment cancellations or rescheduling. Cancellations made within 48 hours or less of your appointment time will incur a fee of 50% of the total appointment charge. ‘No Shows’ are subject to being charged the full appointment fee. I understand that unexpected events can occur and can waive the fee on a case-by-case basis; I do not intend this to be punitive. I appreciate your understanding and cooperation, as providing adequate notice allows my team to offer appointment times to other clients in need. I understand that Dr. Enid F. Martinez is not a licensed medical practitioner in the state of New Jersey. I agree that I am solely responsible for seeking medical advice and treatment for my animal companion from a qualified medical professional when needed.
I agree to actively participate in my pet’s healing process and take personal responsibility for its health and well-being.
